Before submitting your claim, we recommend you read the Ten (10) Factors to Consider when Choosing the Hearing Type before making your selection for the hearing type.

Filing a Claim - No Cost for Consumers

To expedite the handling of your claim, we encourage you to eFile your new claim by clicking on the link above.  Or, you can download, print and complete the applicable claim form below then scan and (i) email it to info@ncdsusa.org, (ii) fax it to 972-807-9919, or (iii) mail it to the address listed on the claim form. Once you submit your claim, NCDS will review your claim and determine if your vehicle and the problem(s) or concern(s) you submitted qualify for the Informal Dispute Settlement Program.
 
Please note, per the rules of our program, we can only accept your claim in English (Tenga en cuenta que, según las reglas de nuestro programa, solo podemos aceptar su reclamo en inglés).

FILE YOUR CLAIM ONLINE TODAY!

Before starting an eFile Claim, you will need your 17 character Vehicle Identification Number or VIN. We also encourage you to have your Repair Orders readily available. After submitting your claim, you will have an opportunity to upload Repair Orders, Repair Receipts, and a Detailed Description of your concern(s).

Hearing Type Notice

If you reside in California and your claim is for a Chrysler, Dodge, Jeep, RAM, Fiat, Alfa Romeo, Tesla, or Toyota vehicle, you can select from one of the following hearing types when completing your claim form:

  1. A Teleconference, Oral Arbitration Hearing*
  2. An In-person, Oral Arbitration Hearing
  3. A Documents-Only, Arbitration Hearing

For all other States and all other vehicle brands in California, you can from one of the following hearing types when completing your claim form:

  1. A Teleconference, Oral Arbitration Hearing*
  2. A Documents-Only, Arbitration Hearing

* The Teleconference, Oral Arbitration Hearing most closely resembles an In-person, Oral Arbitration Hearing in that you can present oral testimony.
